Modification is the act of adding or improving an equipment enhancement using a recipe and the corresponding modification component. See Crafting station

Item quality (color) and character skill affects success rate.

The following list is incomplete and very general. No observable differences could be found for any factor besides item quality (gray, green, blue, purple) and crafting skill pump level. The in-game text lists success/destruction rates for upgrading a modification to rank 5, but actual success at this stage is very limited.
